Saline vs. silicone implants:

Silicone implants were taken off the market by the FDA in the 1990’s when concerns about systemic illnesses such as autoimmune disorders occurred in a  number of patients.  These implants have been redesigned and finally approved by  the FDA after numerous well designed prospective studies showed that silicone implants do not cause these ailments.  Now silicone implants are referred to as “Gels” or “Gummies” in an effort to get away from using the word “Silicone” which still gets negative attention in the media.

Dr. Severinac does not use silicone  implants because he thinks they are dangerous from a health standpoint.  The studies mentioned above have refuted these concerns.  He does not use silicone because saline implants offer a number of significant advantages:

  1.  INCISIONAL SIZE:  The incision for saline implants is less than 1/3 the length needed for silicone implants because during surgery the saline implant is collapsed, placed through the roughly 1 inch incision, and then inflated with a special fill tube that is inserted into a valve built  into the surface of the implant.  Silicone implants are pre-filled and have to be squeezed through the larger incision.
  2. ADUSTABILITY:  Saline implants can be adjusted in terms of precise sizing during surgery. This is very important if a patient has even a slight difference in each breast size.  Even after surgery, saline implants can be adjusted under local anesthesia in the office setting.  Silicone implants are pre-filled and cannot be adjusted.
  3.  MONITORING FOR LEAK:  In the event of an implant leak, the saline implant will simply deflate and the normal saline within the implant will be safely absorbed.  So the monitoring for leak is a simple look in the mirror.  This is not the case with silicone because it is not absorbed.  This is known as the “silent rupture”.  In fact, the FDA highly recommends that patients with silicone implants have MRI scan every other year!  This is a study that insurance would not cover.  It is estimated that there are 150,000 patients in the United States who have a ruptured silicone implant and do not know it.
